Our commitment

Pathways ABA Consultants is required by law to protect the privacy of your protected health information, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your information

We may use and disclose your health information for treatment, payment, and health care operations. For example, we may share information with your child's clinical team to coordinate care, or with your insurer to obtain authorization and payment.

Your rights

You have the right to inspect and request a copy of your records, to request corrections, to request restrictions on certain uses, to request confidential communication, and to receive an accounting of certain disclosures.

Uses that require your authorization

Most uses and disclosures not described in this notice will be made only with your written authorization. You may revoke that authorization in writing at any time.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
